Enrolling at our school
Step 1: Check Your Eligibility
Before submitting an enrolment request, please confirm that your child meets one of the following criteria:
- Lives within the Forest Lake State School catchment area
Check your address using EdMaps – External Link https://www.qgso.qld.gov.au/maps/edmap/ - Has a sibling currently enrolled at Forest Lake State School who will still be attending when your child commences
- Parents wishing to enrol students who are living outside of the catchment area should complete the enrolment enquiry form. The school office will check your child's eligibility in consideration of the school's enrolment plan.
Step 2: Submit an Enrolment Request Online
All enrolment requests must be submitted through our online system Enquiry Tracker.
Please click “Enrolment Request" to complete the online form.
Additional information about the school can be found in the 2026 Enrolment Pack.
You will be asked to provide:
- Your child's details
- Parent/carer contact information
- Birth certificate acknowledge
- Proof of Residency (lease agreement, contract of sale, rates notice)
- Most Recent School Report
- Student Medical Documentation
Please ensure all sections are completed and documents are uploaded correctly, as incomplete requests may delay processing.
Once your request has been verified, you will receive an email containing a link to the Full Enrolment Application.
Step 3: Complete the Full Enrolment Application
After receiving your email invitation:
- Click on the link to complete the full online application form.
- Upload all “Required School Documents"
- State School Media Consent
- SRS Agreement
- BYOD Participation Agreement
- Third Party Online Services Consent Form
Once the enrolment application has been submitted, the following documents must be brought to the school administration for sighting to finalise the application:
Note: Both a passport and a birth certificate are required if the student is on a visa. - Birth Certificate
- Passport
Your enrolment cannot be progressed until all information and documentation has been submitted.

Once your application is complete our enrolments officer will be in contact with you to book an enrolment interview .
