Enrolling at our school is a simple but important process. There are four easy steps to follow.
Check the catchment map
Check the Catchment Map to ensure that you are eligible for enrolment at Forest Lake State School. Parents wishing to enrol students who are living outside of the catchment area should contact the school's office to check their eligibility in consideration of the school's enrolment plan.
The enrolment application
Complete the application form for student enrolment.
A separate application form must be completed for each enrolling student.
Once these are completed, an enrolment meeting with the Principal or Deputy Principal will be arranged at a mutually convenient time.
The Principal considers all applications for out-of-catchment enrolments. Generally, these are approved on legal or medical grounds.

The enrolment interview
The following documents must be available for presentation at the interview. Without the correct documents the enrolment cannot proceed.
Please bring the follow items to the enrolment interview:
Current proof of residency. Examples of this may include a current tenancy agreement, a rates notice or electricity account.
Each child's birth certificate.
A current passport and visa is required for any child born overseas.
Each child’s most recent student report card.
At the enrolment meeting we will:
Discuss the details on your enrolment application.
Organise uniforms, books and any fees payable.
Complete an enrolment agreement.
